Driver Attitude Questionnaire (DAQ)
The DAQ is a 20-item self-report questionnaire designed to measure attitudes regarding a
range of driving behaviors which are collated to identify four factors: drink driving, close following, dangerous overtaking, and speeding. Respondents are required to indicate on a six-point Likert scale (0 = strongly disagree to 5 = strongly agree) their agreement with statements
regarding the appropriateness of various driving behaviors.

Thankyou for your efforts. But I request to give me Driver attitude questionnaire in form of a questionnaire . And can you provide me any tool that asses clients perceptions about counselling sessions or counselor skills.

DBQ (The Manchester Driver Behaviour Questionnaire) is a self-report questionnaire developed by Reason, Manstead, Stradling, Baxter, and Campbell in the United Kingdom in 1990 as a measure of aberrant driving behaviors.
