Dear Alexander,

Thank you for your interest in the cognitive failure questionnaire (CFQ). This is a self-report measure of perception, memory, and motor lapses in daily life. It has 25 items that are scored on a 4-0 scale, with the total score calculated as a sum of the items. You can find more information about the CFQ and its psychometric properties in the references below.

I have attached a copy of the CFQ for your convenience. However, before you use it, you must consider to get permission from the author of the scale. Psychology Roots does not have the right to give permission for using the CFQ. You can contact the author at this email address: d.broadbent@ox.ac.uk.
